Care coordination is the deliberate linking of people, information, decisions, and services so that the client receives one coherent plan rather than a series of disconnected interventions. The client and support network are participants in that plan, not merely recipients of instructions. The CIHC Competency Framework for Advancing Collaboration (2024) describes six interdependent domains: Relationship-Focused Care/Services, Team Communication, Role Clarification and Negotiation, Team Functioning, Team Differences/Disagreements Processing, and Collaborative Leadership. Relationship-focused care and team communication sit centrally because the other domains fail when the team does not understand the client’s goals or cannot exchange information reliably. Application changes with the care setting, the complexity of the situation, and inclusion, access, and equity considerations. For the Canadian RN learner, this content belongs to Safe and Effective Care Environment—Management of Care. The current NCLEX-RN Test Plan took effect on 1 April 2026 and runs through 31 March 2029.
