Quality improvement (QI) and incident reporting answer related but different questions.

Quality improvement (QI) and incident reporting answer related but different questions. QI asks whether a care process reliably produces the intended result and how it can be improved. Incident reporting captures a safety signal: an event or circumstance that could have caused unnecessary harm or did cause it. A report is not a verdict that a nurse made an error. In Canadian patient-safety language, an error is not synonymous with an incident. The same unsafe process may produce a near miss on one shift, a no-harm incident on another, and a harmful incident when circumstances align unfavourably. Reporting all three gives the organization information about system weaknesses before the next patient is injured. The central nursing judgment is sequential: protect the patient first, communicate the event through the right channels, preserve an accurate clinical record, and help the team learn without converting learning into blame.
