Clinical risk identification asks what preventable harm could occur next, for this patient, in this setting, and how the plan should change now. Screening tools organize known predictors, but they do not replace assessment, pattern recognition, or conversation with the patient and family. The RN combines intrinsic vulnerability, treatment hazards, environment, behaviour, communication barriers, and trajectory to choose proportionate controls while preserving autonomy and mobility.
