What the Score Is Designed to Detect
An early warning score is an aggregate weighted track and trigger system.
An early warning score is an aggregate-weighted track-and-trigger system. Each routinely measured physiological variable receives points for deviation from its expected range; the points are added, and the total is linked to a predefined observation frequency and escalation response. The score detects physiological stress. Respiratory failure, shock, sepsis, and neurological injury disturb breathing, oxygenation, circulation, temperature regulation, or consciousness before cardiac arrest occurs. The resulting number is therefore a detection-and-response signal, not a diagnosis. A high score tells you that the patient needs a more urgent assessment; it does not tell you whether the cause is pneumonia, hemorrhage, opioid effect, diabetic emergency, stroke, or raised intracranial pressure. NEWS2 is one commonly used adult system. It is a Royal College of Physicians instrument adopted voluntarily by some hospitals; no single inpatient early warning score is mandated nationally. Your facility may use NEWS2, a modified NEWS2, MEWS, or another validated tool. Use the chart and escalation pathway assigned by your organization.
